Abstract
BACKGROUND Public commentary on the U.S. medical aesthetics market relies heavily on survey extrapolations and repeated secondhand estimates. This analysis measures the market directly from what practices publicly offer and what patients actively seek.
METHODS Cross-sectional observational analysis of 50,000 U.S. aesthetic practices compiled from publicly available sources and structured by Fathom's AI engine. A subset of 26,323 practices carries fully enriched profiles (services, devices, reputation metrics). 503,000+ practice-authored social posts were analyzed for treatment and device mentions. Patient demand was measured as U.S. Google monthly search volume, with January–June 2026 averages compared against a July–December 2025 baseline to control for seasonality.
RESULTS Med spas represent 25.7% of categorized practices; full-service, surgical, dermatologic, and specialty practices constitute the remainder. Botulinum toxin (Botox) remains the most widely offered treatment (21.8% of profiled practices) but its search demand declined 7% in 2026. The largest demand gains occurred in resurfacing and energy-based modalities (broadband light +38%, CO2 laser +30%, RF microneedling +30%) and biostimulatory injectables (Sculptra +25%, Radiesse +17%). GLP-1–related searches (tirzepatide, 1.18M/month, +21%) exceed botulinum toxin searches by a factor of approximately eight.
CONCLUSIONS The composition and momentum of the U.S. aesthetics market differ materially from prevailing commentary. Demand is rotating from neurotoxins and fillers toward skin rebuilding, biostimulation, and weight-management-adjacent services, while supply lags in several high-growth categories.
1. Introduction
Market breakdowns of the U.S. aesthetics industry circulate widely on professional networks, and their headline figures are frequently repeated without verification. Because downstream statistics inherit upstream errors, misstatements about market composition propagate quickly. This report takes a different approach: rather than estimating the market, it observes it, at the level of the individual practice, across 50,000 U.S. aesthetic businesses.
2. Methods
2.1 Data sources
Practice-level data were compiled from publicly available sources (practice websites, business listings, review platforms, and public social media activity) and structured, categorized, and scored by Fathom's AI engine into a proprietary intelligence layer. Of the 50,000 practices in the analysis, 26,323 carry fully enriched profiles including detected services, device inventories, staffing signals, and reputation metrics. In addition, 503,000+ posts authored by these practices on public social channels were analyzed for treatment and device mentions.
2.2 Demand measurement
Patient demand was measured as U.S. Google monthly search volume per treatment term. Because aesthetics search behavior is strongly seasonal (peaking each January), single-month comparisons overstate decline. Trends are therefore computed as the mean monthly volume for January–May 2026 versus the mean for July–December 2025. June 2026 was excluded due to reporting lag.
2.3 Interpretation of counts
Offering counts reflect what practices publicly market, which may differ from clinical volume delivered. Share-of-market figures are reported against categorized practices only; approximately 7,850 profiles could not be confidently categorized and are excluded from composition percentages.

Ratings for plastic surgery and general categories are suppressed pending data-quality review of null-heavy segments. Med spa row merges two source labels.
Principal finding. Med spas, often treated as synonymous with "aesthetics," account for roughly one quarter of categorized practices. Separately, dermatology practices average 478 Google reviews, more than three times the med-spa mean (~150), indicating that review volume rather than star rating is the primary observable reputation differential.

Within the neurotoxin category, for every 100 practices offering Botox, approximately 51 offer Dysport, 24 Xeomin, 14 Jeuveau, and 14 Daxxify. Challenger toxins have achieved meaningful distribution; Section 3.3 indicates they have not achieved comparable patient demand.

Principal finding. Tirzepatide-related search volume (1.18M/month, +21%) is approximately eight times botulinum toxin volume, which declined 7% over the same period. Weight-management demand is currently the largest patient-acquisition current adjacent to aesthetics; 1,600+ practices in the analysis already market hormone, IV, or weight-management services on social channels.

California, Florida, and Texas jointly account for approximately as many practices as the next seven states combined. Relative to population rank, Arizona and Maryland are over-represented, consistent with concentration in sunbelt-retiree and capital-suburb corridors.
4. Discussion
Four claims that circulate widely in industry commentary are inconsistent with the observed data.
- "Aesthetics means med spas." Med spas constitute 25.7% of categorized practices. The majority of the market is full-service, surgical, dermatologic, or specialty, and these segments differ in reputation profile and purchasing behavior.
- "Botulinum toxin demand is accelerating." Botox search demand declined 7% in 2026. Growth concentrated instead in adjacent categories: GLP-1 agents (+21% on an eightfold larger base), resurfacing and energy-based treatments (+29% to +38%), and biostimulatory injectables (+25%).
- "Review scores no longer differentiate practices." Star ratings cluster near the ceiling, but review counts do not: the dermatology mean (478) is more than triple the med-spa mean (~150). Volume, not rating, is the discriminating reputation variable.
- "Body contouring collapsed after the cryolipolysis backlash." CoolSculpting demand is flat rather than declining, roughly 1,500 practices continue to market it, and Emsculpt demand grew 13% in 2026. The category rotated across modalities; aggregate demand persisted.
A consistent pattern across Tables 2 and 3 is a supply–demand mismatch in high-growth categories. - Search volumes are reported in the ad platform's bucketed values; small differences between terms are not meaningful.
- Mean ratings for two categories are withheld pending data-quality review; social-post volume over time is not reported because collection coverage expanded during the study window.
6. Conclusion
Measured directly, the U.S. aesthetics market is larger, more heterogeneous, and differently weighted than prevailing commentary suggests. Demand is rotating toward skin rebuilding, biostimulation, and weight-management-adjacent services while several of those categories remain under-supplied at the practice level. For manufacturers and commercial leaders, the actionable signal is the measurable gap between what patients search for and what nearby practices offer.
